- Contact and appointment request form (on the home page and Contact page): first name, last name, email address, phone number, preferred appointment date, document type, preferred meeting location, any additional details you type in, and your consent to be contacted by email about your request.
- "Get notified" email signup (on the Live Scan and Passport Photos pages): the email address you provide if you choose to be notified when those upcoming services become available.
The contact form asks that you not include sensitive personal information, such as Social Security numbers or financial account numbers, in your message. If your request is urgent, calling or emailing directly is faster and keeps that information off the form.
Information collected automatically
